Why has my SD card stopped working?

Hi,

I have a 8GB SD card, I inserted it on a mp3 player, while I waws listening to a certain song, it suddenly stopped. I turned the player off and turned it on again but no music is played. I tried re-inserting it on my phone but my phone doesn’t read it anymore, even if plugged to pc.

Anyone, help please? : (
