Is there an alternative to Google Docs for creating forms for confession pages on Facebook?

The Indian Government recently banned several websites including Google Docs. I wanted to create a confession page on Facebook and previously used the form feature of Google Docs for that. Now I require another method to create forms for the confession pages. Can you suggest any?
